Each patient is summarized in a structured SBAR format (Situation, Background, Assessment, Recommendation), and pending tasks are handed off to the next clinician with explicit, role-based ownership. You can generate an AI handover snapshot from the patient's live record in one tap — fully editable before you send — and every handover is logged in a per-patient history chain for continuity and audit.

The AI processes voice input only to populate the form, with zero data retention — the audio and transcript are handled transiently and are not stored or retained after processing. Every field stays fully editable before you save, and the saved record lives inside your ClinStreams instance under the same security controls as the rest of your data.

Patient data is isolated per institution, protected by secure authentication, and encrypted in transit and at rest. Every action is captured in an actor-attributed audit log, and the platform is built to HIPAA and GDPR standards.
